Registration Information
In
order to register your child, please bring the following:
1. The student must be present
2. An official document showing proof of birth date. If registering
for Kindergarten, your child must be 5 years old on or before
Sept. 1st, 2008.
3. A GA certificate of immunization (State form #3231)
4. Evidence of vision, hearing, and dental exams obtained in the
past year (State form #3300)
5. Proof of residency in the attendance zone (a utility bill and
a mortgage or lease agreement)*
| School
officials must require the parent or guardian to provide proof
of residency in the district and attendance zone at the time
a student is initially enrolled in a school. Proof of residency
may be verified by presenting a current utility bill with
the same address (excluding telephone bills) and any one of
the following items to school officials: |

A non-contingent sales contract
• Current lease/rental agreement
• Most recent income tax return
• Current paycheck stub
• Current Medicaid card
• Current residential property tax statement or bill
• Current warranty or quick claim deed
• Current home purchase agreement
• Third person affidavit of residency (notarized residency
affidavit)
• Current homeowner's insurance policy. |
6. Proof of custody or guardianship, if not the birth parent
7. Child’s social security number (or you may sign a waiver
at the school)
8. Photo ID for the adult registering the child
Your
child’s registration will be delayed if you do not bring the
necessary paperwork.
*Please note: If you are currently living with
someone in our school zone, you will need a notorized affiidavit.
You may pick this up here at the school. It will then need to be
notorized. We will also need the lease or mortgage agreement of
the
person with whom you are living with as well as a utility bill.
In addition, a picture I.D of the person you are living with will
also be needed.
